Apply for an Other Change to a development approval

Use this form to lodge a request to change an existing development approval where the change involves substantially different development.

Step 1.Gather your documents

Depending on your existing development approval, you may need to submit supporting documentation with your application, such as:

  • State Government forms
  • specialist reports
  • plans and drawings
  • owner consent (mandatory for MCU/ROL).

Download and complete State Government Form 5 and (if applicable) State Government Form 1 and/or 2.

Step 2.Complete and submit our online form

Attach relevant State Government forms and supporting documentation to your online application.

Step 2.Complete the application forms

Attach form/s and supporting documentation to your application.

Step 3.Visit one of our customer service centres

Submit your application and pay at one of our customer service centres Monday to Friday between 8.15am and 4.30pm.
